HGH is the recombinantly produced growth hormone that is identical to the body’s own (somatropin). In randomized studies in healthy older people it measurably changes body composition – a gain in strength or function could not be shown.
What HGH is
Somatropin is supplied directly and raises the levels of growth hormone and IGF-1 strongly and persistently. IGF-1 mediates many anabolic effects in muscle, bone and connective tissue. The body’s own pulsatile release with its feedback loop is bypassed in the process.
The established medical use is in proven growth hormone deficiency. Anti-aging use in people without a deficiency is a different question and rests on a considerably thinner body of data.
The gap between body composition and function
Lean mass and fat mass are surrogate markers: measurable quantities that are meant to stand for something else – here for strength, performance and health in old age. The studies show that this translation cannot be taken for granted.
Body composition changes significantly, weight stays the same, and strength does not increase measurably. These studies provide no data on mortality, fractures or lifespan.
What is well supported
- The effect on body composition is established by meta-analysis. Liu 2007 analyzed 18 study populations of healthy older people (mean age 69 years, 14 micrograms per kilogram per day over a mean of 27 weeks): fat mass minus 2.1 kg (95% CI -2.8 to -1.35), lean mass plus 2.1 kg (1.3-2.9), each with p below 0.001.
- An independent randomized study confirms this in both sexes. Blackman 2002 randomized 131 healthy people aged 65 to 88 double-blind over 26 weeks. Lean mass rose in men by 3.1 kg versus 0.1 kg under placebo (p below 0.001), in women by 1.0 kg versus 0.4 kg (p=0.001).
- Somatropin is an approved medicine with defined indications. The prescribing information (ZOMACTON, label 2024) lists growth disorders in children and replacement in proven growth hormone deficiency in adults. For these uses the substance has been tested and approved.
- The mode of action is fully understood. Somatropin is the recombinantly produced growth hormone that is identical to the body’s own; it raises GH and IGF-1 directly and persistently, and IGF-1 mediates the anabolic effects in muscle, bone and connective tissue. The observed side-effect profile of soft-tissue edema, joint pain and carpal tunnel syndrome corresponds exactly to this mechanism.
What the studies show
Meta-analysis in healthy older people
Liu 2007 analyzed 31 articles on 18 study populations – a total of only 220 people treated with growth hormone, corresponding to 107 person-years. Mean age 69 years, BMI 28, 14 micrograms per kilogram per day over a mean of 27 weeks. Fat mass fell by 2.1 kg (95% CI -2.8 to -1.35), lean mass rose by 2.1 kg (1.3-2.9), each with p below 0.001. Body weight remained unchanged (plus 0.1 kg; -0.7 to 0.8; p=0.87), as did bone density and the other lipids.
Strength: no significant gain
Blackman 2002 randomized 131 healthy people aged 65 to 88 double-blind over 26 weeks. In men, lean mass rose by 3.1 kg under growth hormone versus 0.1 kg under placebo (p below 0.001), and by 4.3 kg with testosterone; in women by 1.0 kg versus 0.4 kg (p=0.001), and by 2.1 kg with hormone replacement. Strength did not increase significantly; the only borderline exception was the combination of growth hormone and testosterone in men (plus 13.5 kg; p=0.05).
Side effects and the authors’ conclusion
In the meta-analysis, soft-tissue edema, joint pain, carpal tunnel syndrome and gynecomastia occurred significantly more often under growth hormone; new-onset diabetes and impaired fasting glucose tended to be more frequent. Total cholesterol fell by 0.29 mmol/l (p=0.006); after adjustment for body composition, this effect was no longer significant. The authors concluded that growth hormone cannot be recommended as an anti-aging therapy in healthy older people.
Where the data stop
- That more lean mass translates into more strength. This is exactly what was tested and not found. Mass increases, strength remains statistically unchanged.
- That HGH extends lifespan. The available studies provide no data whatsoever on mortality, fractures and lifespan – in either direction.
- That the frequently cited Rudman paper of 1990 supports the anti-aging claim. It included 21 men, 12 treated and 9 untreated controls, was not randomized and was short. That is not enough as evidence for long-term therapy.
Status, approval and legal
Somatropin is prescription-only. The prescribing information (ZOMACTON, label 2024) lists exclusively defined growth disorders in children and replacement in proven growth hormone deficiency in adults. An anti-aging indication does not exist; biohacking use is off-label. Somatropin is contraindicated, among other things, in acute critical illness and active malignancy.
In sport, growth hormone is prohibited by WADA. Acquiring it without a prescription is a criminal offense under the German Anti-Doping Act above the quantity specified in the Doping Substances Quantity Ordinance.
Safety
The prescribing information warns of increased mortality in acutely critically ill patients, of second tumors and of reduced insulin sensitivity, especially at higher doses. Added to this are the edema, joint complaints and carpal tunnel syndrome observed in the studies. Products from the gray market that are declared as material “for research purposes only” are not subject to any drug control regarding authenticity and purity.

For growth hormone deficiency and defined syndromes, phase III and registry data from four decades are available – for anti-aging use, nothing comparable. The Rudman paper (NEJM 1990) that is always cited for it included 21 men, 12 treated and 9 untreated controls, was not randomized and was short. In healthy older people, the review by Liu 2007 shows only moderately changed body composition without a gain in function, with more side effects. Known effects at supraphysiological dosing are edema, carpal tunnel syndrome and insulin resistance. Legally, somatropin is prescription-only; acquiring it without a prescription above the quantity specified in the Doping Substances Quantity Ordinance is a criminal offense under the German Anti-Doping Act.
The score rates the state of knowledge, not the substance. “Safety data 9” means well studied – not harmless. “Track record of use 9” means used long and widely – that is not proof of efficacy.

Does HGH really work against aging?
There is no evidence for that. The meta-analysis of randomized studies in healthy older people found changes in body composition, but no data on mortality, fractures or lifespan. The authors concluded that growth hormone cannot be recommended as an anti-aging therapy in healthy older people.
Does HGH build muscle strength?
Mass increases, strength not demonstrably. In the Blackman study, men under growth hormone gained 3.1 kilograms of lean mass versus 0.1 kilograms under placebo. A significant gain in strength did not occur; only the combination with testosterone in men was borderline, with p equal to 0.05.

In the meta-analysis, fat mass fell by 2.1 kilograms and lean mass rose by the same amount. Body weight remained unchanged, at plus 0.1 kilograms with a p-value of 0.87. So it is a shift within the body, not weight loss.
