HGH & longevity: growth hormone fact-checked
August 14, 2026
Growth hormone is the most famous molecule in the anti-aging world, revered as a fountain of youth and feared as a doping agent. The episode shows that the truth is more interesting than hype and panic: real effects, manageable risks and clear rules.
What it is about
HGH is the body’s own build-up and repair signal from the pituitary gland, released in pulses, the largest of them during deep sleep. A large part of its effect works via IGF-1 from the liver. From about 30 onwards, secretion declines; researchers speak of the somatopause. The episode goes from the study that founded the field, via a thymus study with epigenetic clocks, to the risks and a ladder of sleep, training and fasting, peptides and the original. Then there is the history: somatropin has been produced by genetic engineering since 1985; before that, the hormone was obtained from the pituitary glands of deceased people. At the end stands a powerful tool that requires measurement and medical supervision.
Key points
- From about 30 onwards, growth hormone secretion declines by roughly 14 percent per decade; fewer deep-sleep pulses, more belly fat and thinner skin bear this signature.
- In 1990, Rudman gave men over 60 with low IGF-1 growth hormone for 6 months: almost 9 percent more lean mass, a good 14 percent less fat and thicker skin.
- Rudman compared the changes to a reversal of 10 to 20 years of aging; the shift toward less fat and more lean mass has since been confirmed many times.
- In the 2019 TRIIM trial, men received growth hormone with DHEA and metformin for 1 year; the thymus regenerated, and the epigenetic clocks turned back by about 2.5 years.
- In genuine, medically diagnosed growth hormone deficiency, the therapy is officially approved and demonstrably improves quality of life, bone density and blood lipids.
- As a report: users describe better recovery, firmer skin, stronger hair and nails and often deeper sleep.
- In the US, HGH is the only substance whose off-label prescription against aging is expressly prohibited by law; in Germany, somatropin is prescription-only.
Where the data stop
The side effects depend on the dose. Common and usually reversible after dose reduction are water retention, stiff fingers, tingling up to a carpal tunnel sensation and joint aches. HGH counteracts insulin; at higher doses insulin sensitivity can decline, which is why fasting glucose and HbA1c need to be watched. In observational data, persistently very high IGF-1 is associated with slightly more cases of some cancers; the goal is balance in the middle range, not maximization.
TRIIM was small and had no placebo group; a follow-up study is underway, as of August 2026. In Germany, somatropin is covered by statutory health insurance only for diagnosed deficiency; everything else is grey market with a risk of counterfeits, and it is banned in sport. Whether support makes sense is shown by the lab: IGF-1 in the lower age-related third plus matching symptoms are a reason for a conversation with an endocrinologist.
