Magnesium or magnesium L-threonate – is the expensive form worth it?

The short answer

The unique selling point of magnesium L-threonate is the claim that it is the only form that crosses the blood-brain barrier. This claim comes from measurements in rodent brain tissue – in living humans, brain magnesium was not measured in any of the available studies. The two manufacturer-funded studies run for three to six weeks and report, among other things, a cognitive age reduced by 7.5 years as a composite score; in the same series of studies, however, objectively measured sleep values did not differ between the groups. Regular magnesium has solid studies on blood pressure and sleep quality and costs a fraction of the price.

As a cofactor of hundreds of enzymes, very well documented biochemically, with solid studies on blood pressure and sleep quality, but few large outcome trials. Safety well studied; the marketing of individual salt forms goes beyond the comparative data.

Two manufacturer-funded RCTs with 80 and 100 participants over three to six weeks report better sleep quality and a cognitive age reduced by 7.5 years as a composite score – in the same series of studies, however, the objective sleep measurements did not differ between the groups.

Does magnesium L-threonate cross the blood-brain barrier?

In rodents, an increased magnesium content was measured in brain tissue. In humans this has never been checked – neither by imaging nor in cerebrospinal fluid. The central advertising claim is therefore unconfirmed in humans.

Which form of magnesium is best?

For most purposes, tolerability decides, not the form. Magnesium oxide is absorbed less well and tends to have a laxative effect; organic compounds such as citrate or glycinate are better tolerated. Comparative studies between the forms with clinical endpoints are rare.

Does magnesium help with sleep?

There are controlled studies with positive results on sleep quality, but the effects are moderate and the studies mostly small. A deficiency should be ruled out beforehand – then the effect is most plausibly explained.
