Comparison

DHEA (Dehydroepiandrosterone) vs. Pregnenolone

DHEA or pregnenolone – what is really proven for these hormone precursors?

The short answer

Both are the body's own steroid precursors; pregnenolone sits at the start of the chain, and the body makes DHEA from it. DHEA leads on study data: its conversion into testosterone and estradiol has been quantified in humans, it is approved as a vaginal insert in the EU and USA, and in older women lumbar spine bone density rose by 1.0 percent after 12 months. The two-year NEJM study did not confirm the fountain of youth, however. Pregnenolone has small positive studies in low back pain and bipolar depression, but not a single one on memory or hormone balance in healthy people. No study compares the two directly. DHEA is prescription-only in Germany and banned in sport; pregnenolone's status is unresolved. Whether either is an option belongs with a doctor.

The numbers side by side

BK-Score in direct comparison – each axis rates the state of knowledge, not the substance.
AxisDHEA (Dehydroepiandrosterone)PregnenoloneDifference
Human evidenceHow much and how good the data in humans are 7 5 DHEA better studied
MechanismHow well the chain of action is understood 7 6 DHEA better studied
Safety dataHow well safety has been studied – not how safe it is 6 4 DHEA better studied
Hype gap10 means: the advertising stays level with the data 4 3 DHEA closer to the data
Track record of useHow much documented use in humans exists – does not measure whether it works 6 5 DHEA in use longer
RatingSupported, with caveats  ·  Thin human evidence

“Track record of use” measures how long and how widely something has been used in humans – not whether it works. The BK-Score is a subjective assessment by Biohacking Kompakt based on published scoring rules (German). It is not a scientific rating, not the consensus of a professional society and not a medical recommendation.

Why these numbers

DHEA (Dehydroepiandrosterone)

Evidence 7, because there are many randomized studies and several meta-analyses, but the effects are small and inconsistent: sexual function SMD 0.31 in the Cochrane review (Scheffers et al. 2015, 28 studies, 1273 women) versus a narrowly non-significant 0.35 in 23 RCTs (Elraiyah et al. 2014), lumbar spine bone density in women +1.0 % after 12 months (Jankowski et al. 2019, 4 RCTs), and the two-year NEJM study found no effect on body composition, strength, insulin action or quality of life in 87 men and 57 women (Nair et al. 2006); the approval as a vaginal insert (Labrie et al. 2016, phase III) concerns a different use than the oral anti-aging promise. Mechanism 7, because conversion into sex hormones has been quantified in humans (testosterone +24.31 ng/dl, estradiol +7.86 pg/ml, He et al. 2025), but the path to hard endpoints has not. Safety 6, because controlled data over 12 months to 2 years are available, plus approval data for the vaginal form, but no long-term data on the tablet and no data on cancer risk. Hype 4, because DHEA is marketed as a fountain of youth, although precisely this claim did not hold up in the best long-term study. Use 6, because DHEA is freely available as a dietary supplement in the USA, approved as a vaginal insert in the EU and USA, and regulated as prescription-only in Germany.

Pregnenolone

Evidence 5, because although there are a good dozen small randomized studies, they run almost exclusively as add-on therapy in psychiatric diagnoses or pain, with 18 to 120 participants and at most 12 weeks, and their results are inconsistent: low back pain -0.56 points versus placebo (Naylor et al. 2020, 94 veterans), negative symptoms d = 0.79 (Ritsner et al. 2014, 60 patients), but no cognitive effect in the largest study (Marx et al. 2014, 120 participants) and no overall effect in schizophrenia in the meta-analysis (Heringa et al. 2015, k = 4); there is no study on the popular uses in healthy people. Mechanism 6, because the rise of pregnenolone, allopregnanolone and pregnenolone sulfate in serum and an effect on the brain in fMRI have been measured in humans (Sripada et al. 2013), but the NMDA and CB1 trail comes from animal data (Vallée et al. 2014). Safety 4, because all controlled data up to 500 mg per day showed good tolerability, but they only extend to 12 weeks and the hormonal downstream conversion with continuous use has not been systematically measured. Hype 3, because the “mother hormone” is sold as a memory and anti-aging agent, based on mouse experiments with injection into the brain (Flood et al. 1992), while the human data come from entirely different uses. Use 5, because pregnenolone has been sold widely as a dietary supplement for years without authorities in Germany or the USA having clarified its status. Direction mixed: positive individual studies on pain, negative symptoms and bipolar depression, neutral results for cognition and in the meta-analysis.

Frequently asked questions

What is the difference between DHEA and pregnenolone?

Pregnenolone is the first steroid the body builds from cholesterol; it gives rise to progesterone, cortisol, aldosterone and, in two steps, DHEA. DHEA in turn is the precursor from which tissues such as skin, bone and brain make testosterone and oestrogens. Pregnenolone also acts as a neurosteroid in the brain itself. That is why research on pregnenolone has focused mainly on psychiatry and pain, while research on DHEA has focused on hormones, bone and the menopause.

Does the body turn pregnenolone into DHEA?

Biochemically yes, via the enzyme CYP17A1. How much swallowed pregnenolone actually becomes DHEA, sex hormones or cortisol in humans is open, however. A mini-study with 5 men found no change in urinary testosterone ratios after a single dose; systematic measurements with continuous use are lacking. For DHEA, by contrast, the conversion has been measured: in postmenopausal women, testosterone rose by 24.31 ng/dl and estradiol by 7.86 pg/ml.

Do DHEA or pregnenolone work against ageing?

An anti-aging benefit has not been shown for either. The best long-term study on DHEA, two years in the New England Journal of Medicine with 87 men and 57 women, found no effect on body composition, strength, insulin action or quality of life. Pregnenolone's reputation for memory goes back to a 1992 mouse study in which the substance was injected directly into the brain. No controlled study has tested whether swallowed pregnenolone improves memory or hormone balance in healthy people.

Are DHEA and pregnenolone legal in Germany, and do they count as doping?

DHEA is listed as prasterone in the German prescription drug ordinance and is prescription-only; on the 2026 WADA list it is banned at all times under S1.1 and is also named in the German Anti-Doping Act. Pregnenolone is not prescription-only and is sold as a dietary supplement, but whether that is legally sound is unresolved: in 2025 the BVL and BfArM decided that DHEA products are medicines or unauthorised novel foods. Pregnenolone is not on the 2026 WADA list.
