Molecular hydrogen (hydrogen water)

Molecular hydrogen is offered as water with dissolved gas, as tablets that release hydrogen in the glass, or via devices for inhalation. The idea emerged in 2007 with a study in Nature Medicine according to which hydrogen selectively neutralizes the most aggressive oxygen radical. Since then, dozens of small studies in humans have been conducted. They show a few small, measurable effects, mainly on blood lipids and fatigue during exercise, and nothing that supports the anti-aging advertising.

In short

In cell experiments, hydrogen reacted selectively with the hydroxyl radical and left other reactive oxygen species, which the body needs as signals, untouched; in a rat experiment, inhaled hydrogen reduced brain damage after a stroke. In humans with overweight, hydrogen water lowered total cholesterol by 6.71 mg/dl across 13 studies, too little to matter for cardiovascular risk. During exercise, perceived exertion fell somewhat, while endurance and strength stayed the same. The most impressive study concerns inhaled hydrogen after cardiac arrest: it missed its primary endpoint, but the hydrogen group was ahead on survival without lasting damage.

What molecular hydrogen is and how it is supposed to work

What is meant is hydrogen gas, H2, which is dissolved in water, released from tablets or inhaled. It is not the same as alkaline or ionized water, even though devices often promise both. What you should know about drinking water in everyday life is covered in the tip on water quality.

The impetus came from Ohsawa in 2007: in cell cultures under oxidative stress, hydrogen selectively neutralized the hydroxyl radical, the most cytotoxic oxygen radical, but did not react with other reactive oxygen species that have physiological roles. Rats that inhaled hydrogen after an artificially induced stroke had considerably smaller brain damage. Because hydrogen is small and diffuses rapidly through membranes, it is supposed to reach every place where these radicals form. How much of it arrives in the tissue of a person who drinks a glass of hydrogen water, and whether that is enough for an effect, has not been clarified to this day.

What is well supported

For a wellness product, there is an unusually large number of randomized, placebo-controlled studies, and several meta-analyses. In adults with overweight, total cholesterol and LDL fell significantly, if modestly, across 13 studies with 757 participants. In healthy people during exercise, 27 publications with 597 participants showed somewhat more leg jumping power, less perceived exertion and less lactate.

The meta-analyses do not report any safety problems in their summaries.

What the studies show

Ye 2026 – blood lipids in overweight

Meta-analysis of 13 randomized studies with 757 adults with a mean BMI of 25 or higher, mostly with metabolic comorbidities. Hydrogen water lowered total cholesterol by 6.71 mg/dl and LDL by 3.21 mg/dl, HDL fell slightly by 1.16 mg/dl, and triglycerides did not change. The authors consider the effects too small to noticeably influence cardiovascular risk and advise against routine use as a lipid-lowering agent outside of studies.

Zhou 2024 – physical performance in healthy people

27 publications with 597 healthy adults. Maximal oxygen uptake, endurance, anaerobic performance and strength did not change appreciably. Significant and small were leg jumping power (SMD 0.30), perceived exertion (SMD −0.37) and blood lactate (SMD −0.37).

Li 2024 – oxidative stress after exercise

Six studies with 76 participants, using hydrogen water, hydrogen baths or hydrogen gas. The marker of oxidative stress did not differ from placebo (SMD −0.01); the antioxidant potential in the blood rose somewhat (SMD 0.29). Of all things, the core of the advertising, less oxidative stress, could therefore not be shown.

HYBRID II (Tamura 2023) – inhalation after cardiac arrest

73 comatose patients after out-of-hospital cardiac arrest breathed oxygen with 2 percent hydrogen or oxygen alone for 18 hours in 15 Japanese hospitals. A good neurological outcome after 90 days was achieved by 56 versus 39 percent, not significant (p = 0.15). Completely free of symptoms (mRS 0) were 46 versus 21 percent, and 90-day survival was 85 versus 61 percent. The study was stopped early because of the pandemic and was funded by a gas manufacturer. This is intensive care medicine, not a wellness finding.

Where the data stop

The studies are almost all small and short, use different forms, concentrations and amounts, and mostly measure laboratory values instead of health. A result with a particular hydrogen water cannot be transferred to tablets or an inhalation device. A systematic review of 25 papers rates the situation overall as encouraging but preliminary, with a need for larger and methodologically more rigorous studies (Dhillon 2024).

For the most heavily advertised promises, slower aging, more energy in everyday life, detoxification or protection against chronic diseases, there is no study with hard endpoints. The basic mechanism has not been quantified in humans either: the meta-analysis on exercise found no reduction in oxidative stress.

Status, approval and legal

Hydrogen water and hydrogen tablets are sold in Germany as foods; hydrogen as a therapy is not approved for any disease. Inhalation after cardiac arrest has been tested in Japan within studies and is not a standard treatment.

The study results apply only to the form and amount used in each study. Anyone buying a device or tablets should ask how much hydrogen actually ends up in the glass.

Safety

The meta-analyses report no safety problems in their summaries, but they contain no safety analyses of their own, and long-term data are lacking. Hydrogen gas is flammable. Devices that generate hydrogen or mixtures with oxygen for inhalation must be kept away from open flames.

The greater danger is indirect: in elevated blood lipids, diabetes or heart disease, hydrogen water does not replace an evidence-based treatment; the effects in the studies were far below those of established therapies.

The score rates the state of knowledge, not the substance. “Safety data 9” means well studied – not harmless. “Track record of use 9” means used long and widely – that is not proof of efficacy.

Frequently asked questions about molecular hydrogen

What does hydrogen water do?

In studies, it slightly lowered total cholesterol and LDL in people with overweight, by a few mg/dl, and somewhat reduced perceived exertion during exercise. Endurance and strength did not change. There is no evidence for anti-aging, more energy in everyday life or detoxification.

Does hydrogen work as an antioxidant?

In cell experiments, hydrogen selectively neutralized the hydroxyl radical. In humans, this has not been clearly shown: in a meta-analysis on exercise, the marker of oxidative stress did not fall; only the antioxidant potential in the blood rose slightly.

Is hydrogen water the same as alkaline water?

No. Hydrogen water contains dissolved hydrogen gas; alkaline water has a higher pH. Some devices produce both, but the studies on molecular hydrogen refer to the dissolved hydrogen.

What about hydrogen inhalation?

The best-known study tested inhaled hydrogen in 73 patients after cardiac arrest. It missed its primary endpoint; on survival without lasting damage, the hydrogen group was ahead. Nothing can be inferred from this for home devices in healthy people.

Is hydrogen water dangerous?

The meta-analyses do not report safety problems; long-term data are lacking. Hydrogen gas is flammable, and inhalation devices must be kept away from open flames. In lipid metabolism disorders or diabetes, it does not replace treatment.
