Selank is a small synthetic peptide from Moscow that is supposed to relieve anxiety without making people tired or drowsy. It is approved as a medicine in Russia, and unlike most scene peptides it has real studies in patients to show. These studies, however, are small, come almost entirely from Russia and were never tested against placebo.

In brief

Selank is a replica of the endogenous immune peptide tuftsin, stabilized and developed as a nasal spray. In three Russian comparative studies with 60 to 70 patients, it had an anxiolytic effect in anxiety disorders similar to that of benzodiazepines, without their typical drowsiness, and in combination it reduced their side effects. A placebo comparison and independent confirmation outside Russia are lacking, and there are no data for healthy people. Selank is not approved in Germany.

What it is

Selank consists of 7 amino acids. The front part corresponds to tuftsin, a tiny piece of an antibody that occurs in the immune system. At the back there is a small extension that protects the peptide from rapid breakdown, because unmodified, such peptides would often disappear from the body within minutes. Interestingly, then, Selank has immune roots.

It was developed in Moscow, in the same research environment as Semax. The two are regarded as siblings with opposite characters: Semax is supposed to make you alert and sharpen focus, Selank is supposed to calm. In the scene the two are therefore often combined, for clarity without nervousness. That is community practice; there are no studies on the combination.

Why a nasal spray

Peptides hardly stand a chance as a tablet because the stomach breaks them down. Via the nasal mucosa, on the other hand, they travel faster and almost directly toward the brain, bypassing digestion and the liver. That is why both Selank and Semax were developed for use through the nose. In Russia, Selank is registered as over-the-counter nasal drops. An effect is reported just minutes after use.

How it is supposed to work

In the lab, Selank shows two pathways. The fast one runs via the GABA system, the brain’s most important brake, which is also where benzodiazepines act. In rat brain membranes, Selank enhanced the binding of GABA as a so-called positive allosteric modulator. So it turns the fine-tuning knob rather than the main switch, which would fit the described calm without drowsiness. In a rat study, it changed the activity of a whole series of genes involved in GABA transmission, out of 84 genes examined.

The slow pathway runs via BDNF, a growth factor for nerve cells. In a 2019 rat model, Selank influenced BDNF levels in the hippocampus and frontal cortex and prevented alcohol-induced memory impairment. Added to this are indications of effects on serotonin and on enkephalins, the body’s own calming substances, and, from its tuftsin origin, an effect on inflammation-related genes in mice.

Its interplay with classic sedatives was also studied in animals. In rats under chronic unpredictable stress, the combination of Selank and diazepam worked best against the increased anxiety. This fits the patient studies in which Selank performed particularly well as a companion to a benzodiazepine.

What users report

Experience reports from the scene and from Russian use describe an effect that sets in within minutes of spraying: less inner tension, less rumination, but without the cotton-wool, slowed-down feeling that many know from sedatives. “Calm but clear” is the typical verdict. Some also report a slightly lifted mood, which would fit the discussed involvement of serotonin.

In the scene, Selank is framed mainly for phases in which tension gets out of hand but one wants to remain able to work, and mostly as a course of one to two weeks rather than continuous use. These are reports, not proof of efficacy and not a recommendation. They come from a tightly networked circle of users and cannot be separated from expectation effects.

What is well supported

For a scene peptide, Selank has an unusual amount of human data. In three clinical studies from Russia in people with anxiety disorders, it had an anxiolytic effect, measured with established psychiatric scales. In direct comparison with benzodiazepines, the anxiolytic effect was similar, and Selank additionally showed a slightly activating, anti-fatigue effect instead of tiredness.

The combination is particularly interesting: together with the benzodiazepine phenazepam, the effect set in earlier, and typical side effects such as drowsiness and problems with concentration and memory became less frequent, also after the sedative was discontinued. In one study the anxiolytic effect persisted for a week beyond the last dose.

What the studies show

Selank versus medazepam (Zozulia, 2008)

62 patients with generalized anxiety disorder or neurasthenia received either Selank (30) or the benzodiazepine medazepam (32). The anxiolytic effect was comparable; Selank additionally worked against fatigue and was mildly stimulating. The researchers also measured in the blood that the body’s own enkephalins were broken down faster in the patients and normalized under Selank.

Selank versus phenazepam (Medvedev, 2014)

In a comparative study with 60 patients with anxiety and somatoform disorders, Selank showed a pronounced anxiolytic and a mild nootropic effect. The effect persisted one week after the last dose, and the patients’ quality of life improved.

Selank as an add-on to phenazepam (Medvedev, 2015)

70 patients received either phenazepam alone (30) or phenazepam plus Selank (40). In the combination group the treatment worked earlier, and the side effects of the benzodiazepine, including sedation, memory problems and emotional blunting, decreased. This is the study with the most practically interesting finding, because it shows Selank as a companion rather than a replacement.

Where the data stop

Zozulia 2008 and Medvedev 2015 are indexed as randomized; for Medvedev 2014 this is not stated. Blinding is not reported. All three studies compare against a benzodiazepine; none has a placebo arm. Anyone who compares two treatments and sees improvement with both does not know for sure whether both work or whether the symptoms would have improved on their own. The groups were small, 60 to 70 patients, and the papers appeared in Russian-language journals. We found no independent replication outside Russia, and replication is the core of science.

The biohacker dream, Selank as a gentle nootropic for healthy people under stress, has not been studied. The good data come from anxiety patients. The claim that Selank neither causes dependence nor withdrawal also rests on reports and the absence of indications, not on a targeted study in humans. The entire mechanism involving GABA, BDNF and immune genes comes from rats, mice and cell cultures.

What is missing is clearly defined: a blinded, placebo-controlled trial outside Russia, with considerably more patients and follow-up over months. Until then, Selank remains a peptide with real but preliminary human data, more interesting than most scene peptides and yet with an asterisk.

Status, approval and legal

Selank is approved as a medicine in Russia, but not in Germany or the EU. What is offered here is research-grade product from the gray market. Purity and actual content are not pharmaceutically tested; two suppliers can deliver two different products. Its status in sports is not clearly regulated; a review of so-called brain-doping agents describes an unclear classification for many such substances. We do not give dosages for unapproved substances.

Safety

In the patient studies and in experience reports, Selank is regarded as having a friendly profile. The most commonly reported effects are mild nasal irritation from the spray and occasionally tiredness or headache. There are no long-term data in healthy long-term users. Because in the lab Selank contributes to the GABA binding of diazepam and enhanced its effect in rats, a combination with sedatives belongs in medical hands. Persistent anxiety should be assessed by a doctor or psychotherapist.
