Melanotan 1 (afamelanotide): from tanning peptide to medicine

Melanotan 1 began as an idea from Arizona to tan the skin without sun. As afamelanotide, the substance is today an approved medicine that allows people with a rare light disorder to spend more time in sunlight. The episode traces this path.

What it is about

In the 1980s, Victor Hruby and Mac Hadley were looking for built-in protection against skin cancer: more stable copies of the tanning hormone alpha-MSH. Melanotan 1 was the first of these. Unlike Melanotan 2, it acts practically only at the MC1 receptor of the pigment cells, where it stimulates the formation of eumelanin, the brown-black protective pigment, without UV damage being necessary. The Australian company Clinuvel developed it into afamelanotide for erythropoietic protoporphyria, EPP for short, in which just minutes of light trigger attacks of pain lasting for days. The episode goes through the approval studies and looks at vitiligo and the genetics of tanning. At the end stand a medicine administered by a specialist and a grey-market version to which its data do not apply.

  • Melanotan 1 has the same 13 amino acids as alpha-MSH, exchanged at 2 positions, among them norleucine at position 4; this makes it around 10 times more potent and considerably longer-lasting.
  • In 2 randomized phase 3 trials, published in 2015 by Langendonk and colleagues in the New England Journal of Medicine, EPP patients on afamelanotide spent significantly more time in sunlight without attacks of pain.

  • Scenesse is a biodegradable implant the size of a grain of rice that a specialist places under the skin every 2 months and that releases the substance slowly.
  • In vitiligo, afamelanotide combined with narrowband UVB light markedly accelerated repigmentation in studies, especially in darker skin types.
  • With certain variants of the MC1 receptor, typical of redheads and very fair skin types, the body forms more pheomelanin, which protects less well and can itself generate radicals under UV.
  • As a report: users from the grey market describe an even tan and good tolerability, with mild nausea at the start, flushing and occasionally new freckles as the most common side effects.

Where the data stop

The good data apply to the approved implant in EPP, placed and monitored by a specialist. They do not apply to injected Melanotan 1 from the grey market: it is not approved, and purity is a matter of trust. Melanotan 1 is considered the milder molecule and has shown no cancer signal in medical use over years. Nevertheless, any stimulation of the pigment cells can darken existing moles; a skin check beforehand and at regular intervals is part of it, and with many or conspicuous moles it is better to abstain.

The work on further light disorders, sunburn damage, stroke and a cream formulation is research, as of August 2026. Nor does the longevity idea that protected skin ages more slowly replace sun protection: sunscreen remains a must, tan or no tan.
