Omega-3: the fatty acids for a long life fact-checked

August 14, 2026

Omega-3 had already been written off, then in 2025 one of the largest European ageing studies showed a measurably slower ticking of the epigenetic clocks. The episode tells the comeback story of the fatty acids and presents a lab value every biohacker should know.

What it is about

Omega-3 fatty acids are essential: the body cannot make them itself. The important forms EPA and DHA are found mainly in oily sea fish; the plant precursor ALA is converted only to the extent of a few percent. DHA is a building material of nerve cell membranes, and EPA gives rise to inflammation-resolving messengers. The episode tells the state of the research as a drama in 3 acts, from euphoria through disillusionment caused by underdosed studies to the comeback, and explains the omega-3 index as a measure of one’s own status. Added to this are brain, muscles and mood, atrial fibrillation at high doses and the question of quality. What remains is a well-studied, inexpensive foundational supplement with a measurable target value.

Key points

  • In DO-HEALTH, with more than 2,000 people aged 70 and over, 1 gram of omega-3 daily over 3 years slowed biological ageing on several epigenetic clocks by about 3 to 4 months.
  • REDUCE-IT gave high-risk patients 4 grams of highly purified EPA daily and reduced major cardiovascular events by 25 percent; the study was published in 2019.
  • In VITAL, with almost 26,000 participants, the overall result narrowly missed significance; in people who ate little fish, major cardiovascular events fell by 19 percent and heart attacks by 40 percent.
  • A 2021 meta-analysis of 40 studies with 135,000 participants found that the benefit is dose-dependent; many studies without an effect were simply underdosed.
  • The omega-3 index measures EPA and DHA in red blood cells: below 4 percent is considered high-risk, 8 to 11 percent optimal; in Germany most people are at 4 to 6.
  • In our ancestors, omega-6 and omega-3 were in a ratio of about 1:1 to 4:1; the modern diet with vegetable oils is more like 15:1 or higher.
  • As a report: after 2 to 3 months, users report fewer dry eyes, suppler skin and more relaxed joints; less muscle soreness and lower triglycerides are also supported.

Where the data stop

The DO-HEALTH participants were 70 and older, and what was measured were epigenetic clocks, not lifespan itself. The large heart studies mainly show that omega-3 helps those who have too little, and the strong result of REDUCE-IT applies to a high amount of pure EPA in high-risk patients. For the brain, the most consistent data come from observational studies; intervention studies work mainly with a low baseline status and at an early stage.

At high doses, meta-analyses show a moderately increased risk of atrial fibrillation. Anyone with heart rhythm disorders, taking anticoagulants or facing surgery should discuss omega-3 with a physician. A second look at the package pays off: 1 gram of fish oil is not 1 gram of EPA and DHA; often only a third of it is active ingredient.
