Comparison

Orforglipron (Foundayo) vs. Semaglutide (Ozempic / Wegovy)

Orforglipron or semaglutide – which is better supported for weight loss?

The short answer

Both activate the same GLP-1 receptor, and both have been tested in large phase 3 trials. Semaglutide has the deeper data: in STEP 1 weight fell by 14.9 versus 2.4 percent, SELECT with 17,604 people reduced major cardiovascular events from 8.0 to 6.5 percent, and Wegovy has been approved in the EU since 2022. Orforglipron is not a peptide but a small, acid-stable molecule, so it works as a tablet without an absorption enhancer; in ATTAIN-1 weight fell by 11.2 versus 2.1 percent. Hard endpoints are missing; it is approved in the USA, not the EU. The two were compared directly only in type 2 diabetes against the semaglutide tablet, open-label and with blood sugar as the endpoint. Both are medicines; the decision belongs with a doctor.

“Track record of use” measures how long and how widely something has been used in humans – not whether it works. The BK-Score is a subjective assessment by Biohacking Kompakt based on published scoring rules (German). It is not a scientific rating, not the consensus of a professional society and not a medical recommendation.

Why these numbers

Orforglipron (Foundayo)

The phase III trial ATTAIN-1 (NEJM 2025) randomized 3127 participants over 72 weeks: 11.2 percent weight reduction at the highest dose versus 2.1 percent on placebo; the manufacturer cites 12.4 percent for participants who continued treatment. Further phase III trials show a marked HbA1c reduction and superiority over oral semaglutide for blood glucose. As the first orally available non-peptide GLP-1 receptor agonist, its chain of action has been confirmed in humans via trial endpoints; hard cardiovascular endpoints are still pending. The discontinuation rate due to side effects is dose-dependent at 5.3 to 10.3 percent. Approved in the USA since 1 April 2026, not in the EU.

Semaglutide (Ozempic / Wegovy)

Approved on the basis of the STEP trials, cardiovascular benefit in SELECT with a hard endpoint, chain of action quantified in humans, pharmacovigilance ongoing. Deduction on the hype gap for the grey market, which advertises with the same figures without selling the same product.

Frequently asked questions

Is orforglipron better than semaglutide?

For blood sugar, on one measure, yes: in ACHIEVE-3 with 1,698 people with type 2 diabetes on metformin, orforglipron lowered HbA1c more than the semaglutide tablet over 52 weeks, at the highest dose by 1.91 versus 1.47 percentage points. The trial was open-label, and gastrointestinal complaints and discontinuations were more frequent with orforglipron. For weight in obesity there are only indirect comparisons; an analysis funded by the maker of the semaglutide tablet put its highest dose ahead by 3.2 percentage points.

Does orforglipron protect the heart like semaglutide?

That is open. For semaglutide, SELECT shows a hard endpoint: among 17,604 people with existing cardiovascular disease, major events occurred in 6.5 versus 8.0 percent, hazard ratio 0.80, an absolute difference of 1.5 percentage points. This has not been shown for people without prior disease. With orforglipron, waist circumference, systolic blood pressure, triglycerides and non-HDL cholesterol improved in ATTAIN-1, which are risk markers. Outcome trials on heart attack and stroke are still running.

Are orforglipron and semaglutide available in Germany?

Semaglutide is: Wegovy has been approved in the EU since 06.01.2022 and is prescription-only, and since 01.09.2026 Wegovy has also been available as a tablet in German pharmacies. For treating obesity it is not covered by statutory health insurance. Orforglipron is not: the FDA approved it on 1 April 2026 as Foundayo, the UK in August 2026, and in the EU there was no approval recommendation as of September 2026. Online offers of such a new drug carry a high risk of counterfeits.

What side effects do orforglipron and semaglutide have?

With both, gastrointestinal complaints such as nausea, vomiting, diarrhoea and constipation dominate. In ATTAIN-1, 5.3 to 10.3 percent stopped because of side effects; in SELECT, 16.6 percent on semaglutide versus 8.2 percent on placebo did. Trial duration and participants differ, so the figures are not directly comparable. For semaglutide, biliary disease, very rarely NAION, and an FDA warning about thyroid tumours are also known. For orforglipron, safety data so far extend up to 104 weeks.
