Comparison
Coenzyme Q10 (Ubiquinol) vs. PQQ
Coenzyme Q10 or PQQ – what is actually proven for the mitochondria?
The short answer
Coenzyme Q10 has the more robust data. Its role in the respiratory chain is textbook biochemistry, and in heart failure there are hard endpoints: in Q-SYMBIO with 420 patients, 15% of the CoQ10 group reached the composite endpoint over 2 years versus 26% on placebo, plus positive meta-analyses on migraine prophylaxis and sperm parameters. PQQ has three small placebo-controlled studies over 12 weeks showing improvements in sub-domains of memory and attention, two of them with manufacturer authors. Where the data end: the CoQ10 mortality result comes from that single study, and mitochondrial biogenesis by PQQ is only hinted at in humans via one marker. For neither substance is a performance gain in healthy people proven. There is no direct comparison.

“Track record of use” measures how long and how widely something has been used in humans – not whether it works. The BK-Score is a subjective assessment by Biohacking Kompakt based on published scoring rules (German). It is not a scientific rating, not the consensus of a professional society and not a medical recommendation.
Why these numbers
Coenzyme Q10 (Ubiquinol)
Its role in the respiratory chain is established, and blood levels rise reliably after intake. Clinically, the main support comes from Q-SYMBIO (420 patients, 2 years, 15% vs. 26% for the composite endpoint) and the Cochrane review, which rates mortality and hospitalisations as moderate-quality evidence but draws the mortality result from this single study and concludes without convincing overall evidence. Migraine prophylaxis and sperm parameters are positive in meta-analyses. In healthy people, proof of improved performance is lacking; for blood pressure the analyses contradict each other; and for statin-associated muscle symptoms a borderline-significant meta-analysis stands against the most rigorous single RCT.
PQQ
Three placebo-controlled studies with pure PQQ disodium salt (J Am Nutr Assoc 2022 with 64 participants, Adv Exp Med Biol 2016 with 41, Food Funct 2023) found improvements in sub-domains of memory and attention with 20 to 21.5 mg over 12 weeks; all are small, two have manufacturer authors. The 34-person study (J Nutr Health Aging 2024) tested a combination product and missed its primary endpoint in the between-group comparison. Mitochondrial biogenesis has been demonstrated in cell culture; in humans, only the marker PGC-1α rose with training, performance did not (J Am Coll Nutr 2020). EFSA assessed 20 mg per day as safe (margin of safety 344); PQQ has been authorised in the EU since 2018; long-term human data are lacking.
Frequently asked questions
What is the difference between coenzyme Q10 and PQQ?
Coenzyme Q10 is a molecule the body makes itself; in the inner mitochondrial membrane it carries electrons from complexes I and II to complex III – without this carrier, the respiratory chain stops. PQQ is a redox coenzyme used by bacteria and found in foods only in traces. It is meant to trigger the formation of new mitochondria via CREB and PGC-1α; this has been shown in cell culture and animals, in humans only through the rise of one marker.
Which is better proven, coenzyme Q10 or PQQ?
Coenzyme Q10, and with clinical endpoints. The Cochrane review on heart failure rates mortality and hospitalisations as moderate-quality evidence, and for migraine prophylaxis CoQ10 came out at 1.91 fewer attacks per month than placebo in a network meta-analysis. For PQQ there are three small cognition studies over 12 weeks, no meta-analysis and no independent replication. In the BK-Score, human evidence is 7 versus 4.
Do coenzyme Q10 or PQQ give healthy people more energy?
This cannot be demonstrated for either. With CoQ10, blood levels rise reliably, but performance effects in healthy people are small, inconsistent and no longer stable once outliers are excluded; EFSA assessed increased performance as not scientifically substantiated. With PQQ, in a study of 23 untrained men doing endurance training, the marker PGC-1α rose more than on placebo, but aerobic performance did not.
How safe are coenzyme Q10 and PQQ?
For CoQ10, tolerability is well documented; gastrointestinal complaints occur occasionally. Interactions matter more: the interplay with blood pressure medication and coumarin-type anticoagulants is considered insufficiently studied, and cases have been described in which warfarin therapy failed – this belongs in a conversation with your doctor. EFSA assessed PQQ as safe within the authorised framework, with a margin of safety of 344. Long-term data are lacking; pregnant and breastfeeding women are excluded from the authorisation.
