Trenbolone

Trenbolone is an anabolic-androgenic steroid of the nandrolone group. In the US it is approved as an implant for beef cattle, not for humans. In the strength sports scene it is considered one of the most potent anabolic steroids. Precisely this potency has never been measured in humans in a controlled study; the harms, by contrast, show up again and again in surveys and case reports.
In short
There is not a single controlled study on the effects of trenbolone in humans. What circulates about its potency comes from animal and laboratory experiments and from the scene itself. What does exist are observations of users: in a survey of 1,146 men who use steroids, the 237 trenbolone users reported mood swings, irritability and depressive symptoms as well as heart and liver complaints significantly more often than the others. In interviews, users themselves regarded trenbolone as the steroid with the most severe consequences. On top of that come case reports of heart attack, severe cholestasis and pancreatitis in young men. With nandrolone, the question is what a proven effect costs. With trenbolone, even the effect in humans is unproven, while the signs of harm point in the same direction.
What trenbolone is
Trenbolone is a synthetic derivative of testosterone and belongs to the nandrolone group. Regular use exists only in veterinary medicine: in the US, trenbolone acetate is approved as an implant in the ear of beef cattle, for better feed efficiency and only by veterinary order. It is not approved as a medicine for humans.
This makes it fundamentally different from nandrolone or testosterone: for those there are approval studies, prescribing information and decades of medical observation. With trenbolone, this basis is missing. What is said about its effects in humans comes from forums and user reports.
How it is supposed to work
Trenbolone acts via the androgen receptor, that is, at the same switch as testosterone. In the literature it is described as strongly anabolic with limited virilizing effects. This classification is based on animal and laboratory experiments; it has not been measured in humans.
What is certain is what applies to all anabolic-androgenic steroids: the high hormone level signals to the brain that there is enough, the control of the testes is throttled, and the body’s own testosterone production drops. How quickly and how completely it recovers after trenbolone has not been specifically studied.
What is well supported
Little about trenbolone is well supported, and the little that is does not concern its effects. Its approval as a veterinary drug for cattle fattening is documented. It is also documented that trenbolone is widespread in the scene: in the Global Drug Survey 2024, just over one in five men who used steroids had injected trenbolone in the previous year, 237 of 1,146. And a recurring pattern is documented: survey, interviews and case reports independently point in the same direction, towards more mental health problems and more heart and liver complaints.
What the studies show
Bonenti 2026: Global Drug Survey, 1,146 men
The analysis covered men from the Global Drug Survey 2024 who had taken steroids in the previous year, with an average age of 31. 237 had used injectable trenbolone, 909 other steroids without trenbolone. Mood swings, irritability and depressive symptoms were reported significantly more often in the trenbolone group (p < 0.001), as were cardiovascular and liver complaints; the effects were small to medium. Among trenbolone users, complaints also occurred more often in clusters. Limitation: these are self-reported complaints from a survey, not medical findings.
Piatkowski 2023: interviews with users
In a qualitative study, 16 steroid users described their experiences. Of all steroids, trenbolone was regarded as the one with the most severe consequences. Above all, more aggression, violent behavior and difficulty controlling impulses were mentioned; friends and relatives had also noticed the change. This is a description of experiences, not a statement of frequency.
Case reports: heart, liver, pancreas
A 23-year-old bodybuilder came to the emergency department with a heart attack after prolonged use of trenbolone acetate (Shahsavari Nia 2014). A 21-year-old developed severe cholestasis after Parabolan, with bilirubin up to 65.5 mg/dl; because the usual treatment did not help, he was treated with a liver dialysis procedure (Anand 2006). In a 24-year-old, acute pancreatitis recurred after renewed use of trenbolone acetate, which supports the association (Kumar 2019). Case reports show what can happen, not how often.
Borecki 2024: review of the organ effects
A review summarizes the described side effects: severe acne and breast gland enlargement in about one third of users, plus increased hair growth, stretch marks, high blood pressure and cardiac arrhythmias, as well as inflammation and tissue damage at the injection site from contaminated preparations. The authors stress that many effects are unknown, partly because several steroids are often taken at the same time.
Where the data stop
- No efficacy study in humans. There is no controlled study on either muscle mass or strength. The widespread view of it as a particularly potent steroid comes from animal experiments and scene knowledge.
- No long-term data. How the heart, liver, blood lipids and hormonal axis behave over years has not been studied for trenbolone. The large registry data concern steroids as a whole, see Anabolic steroids.
- Self-report and polydrug use. The survey measures what users report, not what a doctor finds. And anyone who takes trenbolone often takes other steroids too; cause and effect can hardly be separated this way.
- Origin of the products. There is no human medicine. Everything traded for humans comes from the gray market or black market; content and purity are unknown.
Status, approval and legal
Trenbolone is not approved as a medicine for humans. In the US, trenbolone acetate is approved as an implant for beef cattle, only by veterinary order. In Germany, finished medicinal products may only be placed on the market with an approval (§ 21 of the German Medicinal Products Act, Arzneimittelgesetz).
Trenbolone is listed by name in the annex of the German Anti-Doping Act. Manufacture, trade and supply for doping purposes are prohibited, as are acquisition, possession and bringing into or through Germany of a not insignificant quantity for doping in sport; violations are criminal offenses. In sport it is on the WADA Prohibited List under S1.1 and is banned at all times, in competition and in training.
Safety
Mental health: mood swings, irritability and depressive symptoms were reported significantly more often by trenbolone users than by other steroid users; in interviews, users described aggression, violence and loss of impulse control. Heart and blood vessels: cardiovascular complaints were reported more often; high blood pressure, cardiac arrhythmias and a heart attack in a 23-year-old have been described. Liver: liver complaints were reported more often; a case of severe cholestasis is documented. Pancreas: a recurrent acute inflammation after renewed use. Hormonal: shutdown of the body’s own testosterone production as with all anabolic-androgenic steroids, plus breast gland enlargement and severe acne in about one third. Blood lipids: measurements specific to trenbolone are lacking; for oral steroids such as oxandrolone, falling HDL and rising LDL have been measured in studies.
The authors of the case reports stress how important it is that doctors know about steroid use. Anyone who has taken steroids and notices problems with the heart, liver or mental health belongs in medical hands, openly and without concealing the substance.

Is trenbolone banned in Germany?
It is listed in the annex of the German Anti-Doping Act. Trade and supply for doping purposes as well as acquisition and possession of a not insignificant quantity for doping in sport are prohibited and punishable. In sport it is on the WADA Prohibited List and is banned at all times.
