Question & Context

Low desire, low drive – is it testosterone?

In brief

Sometimes yes, often not alone. A real testosterone deficiency shows most clearly in three sexual signs: less desire, fewer morning erections and erection problems. Tiredness and lack of drive alone are too unspecific for this. Whether it is the hormone can only be clarified by a repeated morning blood test together with the symptoms. The biggest levers in your own hands are sleep, body fat and training; with a confirmed deficiency, medically supervised testosterone therapy is well studied, while herbal boosters have so far delivered little that is reliable.

What matters first

Have it measured properly

A single value from the afternoon says little, because testosterone has a pronounced daily rhythm with its peak in the morning. The Endocrine Society guideline recommends measuring total testosterone in the morning while fasting and confirming a low value with a second measurement. A diagnosis is only made when matching symptoms and clearly low values come together. If a deficiency is confirmed, the search for the cause is part of it too.

Protect your sleep

Sleep is the most directly measured lever. In a lab study of ten healthy young men, daytime testosterone after eight nights of five hours of sleep each was 16.5 instead of 18.4 nanomoles per liter. The group was small, but the direction was clear: if you sleep too little over the long term, you pay for it in hormones and in drive.

Reduce body fat

Fat tissue converts testosterone into estradiol and thus slows down the body’s own production. A meta-analysis of 24 studies found an increase in total testosterone of 2.87 nanomoles per liter with a calorie-reduced diet, and of 8.73 after stomach surgery. The more weight the men lost, the more the value rose.

Strength training as the foundation

Strength training does not raise testosterone through the short hormone surge after the session; that has been experimentally refuted. It works indirectly: more muscle, less body fat, better metabolism. This includes a diet that is not too low in fat; in six intervention studies with 206 participants, a very low-fat diet lowered total and free testosterone.

When you should have this checked by a doctor

See a doctor if your desire drops noticeably over weeks, morning erections stop or erection problems newly appear. Erectile dysfunction is not just a hormone issue but often an early vascular signal: in a meta-analysis of 25 studies with 154,794 men, the risk of cardiovascular disease was 43 percent higher in men with erectile dysfunction. You should also promptly have the following checked: painful swelling of the breast, changes in the testicles, newly occurring headaches or visual disturbances, and persistently low mood. Good points of contact are your family doctor, urology or endocrinology, and gynecology for women.

What testosterone explains and what it does not

The most important study on the question comes from Europe: 3,369 men between 40 and 79 years of age were surveyed and measured in the morning. Tiredness, low mood and reduced performance were associated with lower values, but only the three sexual signs formed a distinct clinical picture. The authors therefore proposed assuming an age-related deficiency only with at least three sexual symptoms together with total testosterone below 11 nanomoles per liter and free testosterone below 220 picomoles per liter.

This fits the therapy studies. In the American T-Trials, 790 men aged 65 and over with low values and matching symptoms received testosterone gel or placebo for one year. Sexual function improved moderately, mood somewhat; for vitality and walking distance, there was no benefit. If you are mainly exhausted, you should therefore also look at sleep, stress, weight, iron status and thyroid.

When therapy comes into play

If a deficiency is confirmed, therapy belongs in medical hands. The large TRAVERSE trial with 5,246 men with a deficiency and increased cardiovascular risk showed that testosterone gel does not cause serious cardiovascular events more often than placebo; atrial fibrillation, acute kidney injury and pulmonary embolism, on the other hand, occurred more often. In 2025, the US Food and Drug Administration (FDA) also introduced a warning about increased blood pressure for all testosterone products.

One point is often overlooked: testosterone supplied from outside suppresses the body’s own sperm production. The guideline therefore advises against starting therapy if you wish to have children in the near future. This is exactly where the interest in substances such as enclomiphene comes in, which are meant to stimulate the body’s own production.

And in women?

In women, too, testosterone plays a role in desire, but a blood value alone cannot establish a desire disorder. An international consensus statement by eleven professional societies sees exactly one supported use: women after menopause in whom a desire disorder has been diagnosed after an assessment of body, mind and relationship. In this group, testosterone in physiological doses brought on average about one additional satisfying sexual event per month. For younger women and for general well-being, the data are not sufficient.

What the database says

Sorted by strength of evidence. The BK-Score rates the state of knowledge, not the substance.

  • Testosterone & TRT Evidence 9/10 · Well supported, heavily overhypedMedical testosterone therapy for a diagnosed deficiency: approved for decades, very well studied in humans and with a known side-effect profile that makes monitoring necessary.
  • Strength training 3× per week Evidence 9/10 · Well supportedStrength training is one of the best-supported measures in the database. For desire, it works indirectly via body composition, metabolism and self-efficacy.
  • Sleep & sleep hygiene Evidence 7/10 · Supported, with caveatsSleep is the basis for the morning rise in testosterone and for drive during the day. The page shows what regularity and sleep hygiene can do.
  • Zinc Evidence 7/10 · Supported, with caveatsZinc raises testosterone again in a real deficiency; with normal zinc status, no effect has been shown. A topic for people with a one-sided diet or increased needs.
  • Enclomiphene Evidence 7/10 · Supported, with caveatsIn studies, it raises the body’s own production while preserving fertility. What is supported are lab values, not a clinical benefit, and the substance is not approved in Germany.
  • Tongkat Ali Evidence 6/10 · Supported, with caveatsThe root extract with the best data among the herbal boosters: a meta-analysis of five randomized trials sees an increase, especially with a low starting value. It is not approved in the EU.
  • Raising testosterone naturally Evidence 5/10 · Thin human evidenceThe overview of the building blocks without a prescription, sorted by strength of evidence: sleep, body fat and dietary fat with human data, zinc only in deficiency, boron and magnesium without robust evidence.
  • Ashwagandha Evidence 5/10 · Thin human evidenceFits rather with the stress side of drive: small studies show less perceived stress and lower cortisol levels over eight weeks. The BfR warned of rare liver damage in 2024.
  • Tribulus Terrestris Evidence 5/10 · Studied – no benefit shownThe classic libido herb. A review of ten studies found no robust testosterone effect, and only weak indications for erections.

Frequently asked questions

At what level is testosterone considered too low?

There is no fixed limit for everyone; what matters is the combination of symptoms and repeatedly low morning values. For age-related deficiency, the large European EMAS study proposed total testosterone below 11 nanomoles per liter together with at least three sexual symptoms. Laboratory limits can differ depending on the measurement method, so the assessment belongs in medical hands.

Can I raise my testosterone level without medication?

Yes, especially if lack of sleep or excess weight play a role. More sleep, less body fat and a diet that is not too low in fat have shown measurable effects in studies. Strength training supports this via body composition. In real hypogonadism, however, these steps do not replace a diagnosis or therapy.

Do testosterone boosters from the internet work?

Most have not been shown to. For tribulus, a review found no robust effect; for boron, the only randomized trial in men showed nothing; and for Fadogia agrestis there is not a single human study. The best indications exist for tongkat ali with a low starting value, but the extract is not approved in the EU.

Why do I have no desire despite normal values?

Desire arises in the mind and in the whole body, not just in the hormone lab. Stress, exhaustion, relationship issues, depressed mood and some medications can markedly dampen libido. An open conversation at your family doctor’s practice is a good first step, if needed also with support from sexual medicine or psychotherapy.

Is testosterone therapy dangerous for the heart?

The largest study on this to date, TRAVERSE, found no increase in serious cardiovascular events in men with a deficiency and increased risk. However, atrial fibrillation, acute kidney injury and pulmonary embolism occurred more often, and the FDA now warns of a rise in blood pressure. Therapy therefore requires a clear diagnosis and regular check-ups.

Should I measure testosterone first if I have erection problems?

Testosterone is part of the assessment, but is often not the main cause. Erectile dysfunction is closely linked to vascular health and can occur years before a heart attack. That is why it is worth checking blood pressure, blood sugar and blood lipids together with hormone status.
