Orforglipron: the weight-loss pill fact-checked

The weight-loss injections have shown that the satiety system can be steered. Orforglipron brings this into a tablet with no fridge and no needle, approved in the US since April 2026, and behind it lies a small feat of chemistry.
What it is about
Orforglipron targets the same receptor as semaglutide and tirzepatide, the GLP-1 receptor, but it is not a peptide; it is a small, acid-stable molecule. For a long time this receptor was considered too complex for such small molecules; chemists at the Japanese company Chugai found one, and Eli Lilly licensed it in 2018. The episode explains the advantages: swallowed daily without fasting rules, no cold chain, conventional tablet production. Then it covers the studies on obesity and type 2 diabetes, side effects, muscle protection and the question of what happens after stopping. At the end stands not a stronger but a more practical drug of the GLP-1 family, which is still awaiting approval in Europe.
Key points
- In ATTAIN-1, published in 2025 in the New England Journal of Medicine, more than 3,000 adults with obesity lost a good 12 percent of their weight in 72 weeks on the highest dose, about 12.5 kilos, compared with around 1 kilo on placebo.
- This puts the tablet somewhat below the semaglutide injection at around 15 percent and clearly below tirzepatide at more than 20 percent; blood pressure, blood lipids and inflammation markers improved measurably.
- In the ACHIEVE trial series in type 2 diabetes, long-term blood sugar (HbA1c) fell by up to 1.5 percentage points, along with around 8 kilos of weight loss.
- The semaglutide tablet Rybelsus must be taken on an empty stomach, followed by a 30-minute wait, and only about 1 percent of the substance is absorbed; orforglipron is absorbed normally.
- In April 2026, US approval came under the name Foundayo, for obesity and for overweight with a comorbidity, as the world’s first non-peptide GLP-1 tablet.
- According to surveys, around half of candidates reject long-term therapy with a needle from the outset; it is exactly these people the tablet could reach.
Where the data stop
The side effects are those of the class: mainly nausea, constipation, diarrhea and occasionally vomiting, mostly during the dose-escalation phases. It is not for use in pregnancy, and caution applies with a history of pancreatic disease. The cardiovascular markers point in the right direction, but studies with actual heart attack numbers are still ongoing. After stopping, without further changes, much of the weight returns over 1 to 2 years; strength training, protein and eating routines are what remain.
In the EU, orforglipron is not approved, as of August 2026; the procedure at the European Medicines Agency (EMA) is ongoing. Offers from dubious internet sources are particularly prone to counterfeiting for a brand-new substance. And a daily tablet is easier to forget than one injection a week.
