Tesofensine – the appetite off switch fact-checked

Tesofensine was intended as a drug for Parkinson’s disease and failed, but the test subjects lost weight in the process. The episode tells how it became a weight-loss drug in tablet form and why the heart plays the decisive role.
What it is about
Tesofensine is a triple reuptake inhibitor: it keeps dopamine, noradrenaline and serotonin active for longer, precisely the messengers that control appetite, drive and reward. Hunger becomes quieter, you feel full sooner, and there is also a small boost to metabolism. The episode goes through the central TIPO-1 study and explains the catch: pulse and blood pressure rise slightly. The elegant answer to this is Tesomet, the combination with the beta blocker metoprolol. What remains is a strong candidate that could offer a tablet to people who do not want an injection, but which is not approved in Germany and belongs in a physician’s hands.

Where the data stop
The effect on the heart and circulation is the reason why tesofensine was never approved as a stand-alone drug. Because it acts on dopamine and serotonin, it can affect mood, and it is not for people taking psychiatric medication or prone to restlessness and anxiety. Long-term courses in humans are less well studied than with the established injections, and after stopping, hunger returns.
In Mexico, the partner behind tesofensine has applied for approval, and the authority has responded positively; in the US and in Germany it is not approved, as of July 2026. Gray-market products are particularly tricky with such small active amounts, because even small deviations multiply the dose. Protein and strength training remain necessary so that not too much muscle is lost during weight loss.
